To assist you in encouraging your patients to plan for the future, Honoring Choices® Idaho has assembled a multitude of resources for physicians, nurses, social workers, chaplains and other health care professionals in Idaho.
Through a contractual relationship with C-TAC Innovation’s Respecting Choices®, Honoring Choices® Idaho is establishing a cadre of trained instructors to certify individuals to facilitate advance care planning conversations in a variety of Idaho settings (i.e. clinics, community based organizations, faith groups).
